The same thing the Jays' pitcher and catcher were doing. All thinking there were two outs. I'm guessing it was the ump's mistake because they tend to suck.

Red Sox forgot how many outs there was so it was a double play. However, since it is a “timing” play and the Blue Jays threw to first base for third out and never appealed Seigler leaving third base early and left the field, the run counts and it’s ruled a sac fly for Rafaela.

Blue Jays scoreboard was showing two outs, which probably lead to the confusion with the Sox baserunners.  It’s pretty rare to have two guys on base both make that same mistake simultaneously…
